Anthem PPO Plan

The Anthem PPO plan offers you the freedom to go in and out of the network for care. You may use a PPO provider whose negotiated rates provide richer levels of benefits, with claim forms filed by the provider. You may also obtain services outside the PPO network; however, you will incur higher out of pocket expenses and you are responsible for filing claims.

Anthem High Deductible Health Plan (HDHP)

The Anthem High Deductible Health Plan is a traditional PPO medical plan which offers provider choice with a higher deductible. It includes an employer contribution of $600 to the Health Savings Account (HSA), which you can use to satisfy the deductible, save money or pay for other eligible health care expenses. Use your HSA to pay for Healthcare costs, such as deductible payments, hospital fees, etc. For a full list of qualified medical expenses, please visit anthem.com/ca/qme.

You can contribute up to $4,300 for individuals or $8,550 for families.
