Enrollment Option 1: Telephonic Enrollment with a Benefit Counselor​

We are pleased to announce that you may speak with a licensed, professional Benefit Counselor. The Counselor will explain the benefits, help you decide on the right benefits for your situation and budget, answer your questions, and complete your enrollment in one easy phone call. If you like, we can even conference in your spouse! All you need to do is pre-schedule your phone appointment.

Schedule as soon as possible for the best choice of day and time. Appointments will fill up fast.

Please be prepared for your enrollment session. Remember to have the following information available when you enroll:

• Dependent info if enrolling dependents
  •  Name
  • Social Security #
  • Date of birth
• Beneficiary info for life insurance
  • Name
  • Relationship

To Waive All Benefits

We encourage you to review your benefit choices each year to be sure they are meeting your needs. However, we understand that you may have coverage elsewhere, through a spouse, etc. Please complete the paper waiver form, sign it and return it to your manager.
